Disjointed, corrupted, blocky or blank video when using High Input Performance Mode

Certain inputs such as video, camera and NDI may look look disjointed or corrupt with a large amount of black space at the bottom of the preview like the following:
 
 
Video inputs may also appear blank with only audio working.
 
This is caused by an incorrect setting in the NVIDIA Control Panel. To fix this:
 
1. Open up the NVIDIA Control Panel.
2. Select Manage 3D settings
3. Change the Anti-Aliasing mode in the Global Settings list to "Application-controlled" as shown below and click Apply.
4. Also check to make sure the Antialiasing - FXAA option is set to Off
